	&lt;p&gt;I.  REFERENCE&lt;/p&gt;
	&lt;p&gt;    Business and Finance Bulletin G-28, "Policy and Regulations Governing&lt;br&gt;
    Travel."&lt;/p&gt;
	&lt;p&gt;II.  INTRODUCTION&lt;/p&gt;
	&lt;p&gt;     A.  Purpose&lt;/p&gt;
	&lt;p&gt;         This Bulletin outlines the University's Business Travel Accident&lt;br&gt;
         Insurance Program for eligible individuals traveling on University&lt;br&gt;
         business.  The program provides accidental death and dismemberment&lt;br&gt;
         insurance coverage for eligible individuals traveling on&lt;br&gt;
         official University business.  There is also coverage provided for&lt;br&gt;
         specified eligible individuals while engaged in certain hazardous&lt;br&gt;
         activities.&lt;/p&gt;
	&lt;p&gt;     B.  Eligibility for Insurance Coverage&lt;/p&gt;
	&lt;p&gt;         For the specific purpose of the Business Travel Accident Insurance&lt;br&gt;
         Program, the following individuals are eligible for coverage:&lt;/p&gt;
	&lt;p&gt;         1.  Regents and Regents Designate of the University of California&lt;br&gt;
             (except as referred to under Section V.B., below).&lt;/p&gt;
	&lt;p&gt;         2.  All employees of the University.&lt;/p&gt;
	&lt;p&gt;         3.  Interns, residents, and fellows associated with the University&lt;br&gt;
             in connection with emergency medical programs.&lt;/p&gt;
	&lt;p&gt;         4.  Professors emeriti traveling on official University business.&lt;/p&gt;
	&lt;p&gt;         5.  Faculty on approved sabbatical leave when required to travel as&lt;br&gt;
             part of their program of research or study.&lt;/p&gt;
	&lt;p&gt;         Employees at the Los Alamos National Scientific Laboratory are&lt;br&gt;
         insured under a separate program administered by the Los Alamos&lt;br&gt;
         Laboratory.&lt;/p&gt;
	&lt;p&gt;III.     COVERAGE&lt;/p&gt;
	&lt;p&gt;         An eligible individual will be covered 24 hours a day, worldwide,&lt;br&gt;
         against accidental death and dismemberment for up to $100,000&lt;br&gt;
         (called the Principal Sum) while traveling on official University&lt;br&gt;
         business or while engaged in designated hazardous activities on&lt;br&gt;
         behalf of the University (the Principal Sum for Executive Program&lt;br&gt;
         members is $250,000).&lt;/p&gt;
	&lt;p&gt;         A.  Business Trips&lt;/p&gt;
	&lt;p&gt;             For coverage purposes, an official University business trip&lt;br&gt;
             begins when the eligible individual leaves his/her residence or&lt;br&gt;
             work site, whichever occurs last, for the purpose of conducting&lt;br&gt;
             business away from the work site and ends when he/she returns&lt;br&gt;
             to his/her residence or work site, whichever occurs first.  If&lt;br&gt;
             a business trip exceeds 60 days in length, the eligible&lt;br&gt;
             individual will be considered to be located at an alternate&lt;br&gt;
             residence and work site and must be on a trip away from the&lt;br&gt;
             alternate sites for coverage to apply.&lt;/p&gt;
	&lt;p&gt;             Business trips on behalf of the University will include&lt;br&gt;
             personal deviations which occur during and which interrupt the&lt;br&gt;
             continuity of an otherwise authorized business trip, but&lt;br&gt;
             do not include any loss which occurs during a vacation period&lt;br&gt;
             or leave of absence.&lt;/p&gt;
	&lt;p&gt;             Procedures and conditions of travel must be in accordance with&lt;br&gt;
             Business and Finance Bulletin G-28 which describes the&lt;br&gt;
             University policy and regulations regarding travel.&lt;/p&gt;
	&lt;p&gt;          B. Designated Hazardous Activities&lt;/p&gt;
	&lt;p&gt;             1. Hazardous activities undertaken at the direction of the&lt;br&gt;
                University covered by this program are:&lt;/p&gt;
	&lt;p&gt;                a.  Transportation of emergency medical patients or donor&lt;br&gt;
                    organs.&lt;/p&gt;
	&lt;p&gt;                b.  Structural inspection.&lt;/p&gt;
	&lt;p&gt;                c.  Scuba diving.&lt;/p&gt;
	&lt;p&gt;                d.  Seismology and wave studies.&lt;/p&gt;
	&lt;p&gt;                e.  Hazardous spills cleanup.&lt;/p&gt;
	&lt;p&gt;                f.  Authorized activities of U.C. Police Bomb Unit.&lt;/p&gt;
	&lt;p&gt;                These activities are covered on and off University premises.&lt;/p&gt;
	&lt;p&gt;             2. Coverage while an eligible individual is in command of a&lt;br&gt;
                general aviation aircraft will be provided only if the&lt;br&gt;
                eligible individual is approved for such travel and in full&lt;br&gt;
                compliance with University regulations in accordance with&lt;br&gt;
                Business and Finance Bulletin G-28, "Policy and Regulations&lt;br&gt;
                Governing Travel."&lt;/p&gt;
	&lt;p&gt;             3. Coverage is provided for war risk hazards outside the United&lt;br&gt;
                States in areas designated by the respective insurance&lt;br&gt;
                carrier as war risk areas.  This provision covers claims&lt;br&gt;
                caused or contributed to by war (whether direct or indirect),&lt;br&gt;
                hostilities or war-like operations (whether or not war was&lt;br&gt;
                declared), invasion, act of foreign enemy, civil war,&lt;br&gt;
                rebellion, revolution, insurrection, or military or usurped&lt;br&gt;
                power.&lt;/p&gt;
	&lt;p&gt;             4. Coverage is provided for terrorist acts committed in the&lt;br&gt;
                Unites States regardless of whether they are also considered&lt;br&gt;
                to be acts of war.&lt;/p&gt;
	&lt;p&gt;             5. In compliance with Libyan Sanction Regulations, 31 C.F.R.&lt;br&gt;
                Part 550, insurance is not provided in Libya, as this would&lt;br&gt;
                violate Section 550.202 of those regulations.&lt;/p&gt;
	&lt;p&gt;IV.    EXCLUSIONS&lt;/p&gt;
	&lt;p&gt;       The business travel policy does not cover any loss, fatal or&lt;br&gt;
       non-fatal, caused by or resulting from the following:&lt;/p&gt;
	&lt;p&gt;       A.  Suicide, attempted suicide, attempted self-destruction, or&lt;br&gt;
           self-destruction while sane or insane.&lt;/p&gt;
	&lt;p&gt;       B.  Disease of any kind.&lt;/p&gt;
	&lt;p&gt;       C.  Bacterial infections except certain infections which occur&lt;br&gt;
           through an accidental cut or wound.&lt;/p&gt;
	&lt;p&gt;       D.  Declared or undeclared war or any act thereof in the United&lt;br&gt;
           States or outside the scope of the purchased war risk coverage.&lt;/p&gt;
	&lt;p&gt;       E.  Service in the military, naval, or air service of any country.&lt;/p&gt;
	&lt;p&gt;       F.  Riding as a passenger or otherwise in any vehicle or device for&lt;br&gt;
           aerial navigation except:&lt;/p&gt;
	&lt;p&gt;           1.  Any civilian aircraft having a current and valid&lt;br&gt;
               airworthiness certificate and piloted by an individual who&lt;br&gt;
               holds a valid and current certificate of competency or a&lt;br&gt;
               rating authorizing that individual to pilot such aircraft.&lt;/p&gt;
	&lt;p&gt;           2.  Any transport type aircraft operated by the United States&lt;br&gt;
               Armed Forces or similar service of any recognized country.&lt;/p&gt;
	&lt;p&gt;           3.  Those aircraft owned or leased by the University which have&lt;br&gt;
               been specifically included for coverage.&lt;/p&gt;
	&lt;p&gt;V. BENEFITS&lt;/p&gt;
	&lt;p&gt;   A.  Accidental Death and Dismemberment Indemnity&lt;/p&gt;
	&lt;p&gt;       If injury results in any one of the specific losses shown in the&lt;br&gt;
       table below, the insurer will pay, with respect to each insured, the&lt;br&gt;
       applicable benefit based on the Principal Sum, provided that not more&lt;br&gt;
       than one (the largest) of such benefits will be paid with respect to&lt;br&gt;
       injuries resulting from one accident and no benefits will be paid&lt;br&gt;
       under more than one of the policy hazards descriptions as a result of&lt;br&gt;
       one accident.&lt;/p&gt;

	&lt;p&gt;       "Member" means hand, foot, or eye.  "Loss" means, with regard to hand&lt;br&gt;
       or foot, actual severance through or above the wrist or ankle joints;&lt;br&gt;
       with regard to eye, entire and irrecoverable loss of sight; with&lt;br&gt;
       regard to thumb and index finger, actual severance through or above&lt;br&gt;
       metacarpophalangeal joints; with regard to speech, entire and&lt;br&gt;
       irrecoverable loss of speech; with regard to hearing, entire and&lt;br&gt;
       irrecoverable loss of hearing in both ears.  "Loss of four fingers of&lt;br&gt;
       the same hand" means loss by actual severance through or above the&lt;br&gt;
       metacarpophalangeal joints.  "Loss of four toes of the&lt;br&gt;
       same foot" means loss by actual severance through or above the&lt;br&gt;
       metatarsophalangeal joints.&lt;/p&gt;
	&lt;p&gt;       The total amount payable for one accident regardless of the number of&lt;br&gt;
       eligible individuals involved is $4,000,000.&lt;/p&gt;
	&lt;p&gt;   B. Permanent Total Disability Benefit&lt;/p&gt;
	&lt;p&gt;   This benefit is available for all individuals eligible for coverage&lt;br&gt;
   excluding individual Regents and Regents Designate.  The major points&lt;br&gt;
   are:&lt;/p&gt;
	&lt;p&gt;      1.  Permanent Total Disability is defined as the permanent inability&lt;br&gt;
          to engage in each and every occupation for compensation or profit&lt;br&gt;
          for which the individual is reasonably qualified by reason of&lt;br&gt;
          education, training, or experience.&lt;/p&gt;
	&lt;p&gt;      2.  The period of Permanent Total Disability must result from an&lt;br&gt;
          injury sustained while on covered University travel or while&lt;br&gt;
          engaging in covered extra-hazardous activities.&lt;/p&gt;
	&lt;p&gt;      3.  The Permanent Total Disability period must begin within 120 days&lt;br&gt;
          of the covered injury.&lt;/p&gt;
	&lt;p&gt;      4.  The Permanent Total Disability must continue for a period of&lt;br&gt;
          twelve consecutive months before benefits are payable.&lt;/p&gt;
	&lt;p&gt;      5.  The benefit level payable is 1 percent of the Principal Sum per&lt;br&gt;
          month less any amounts payable under the dismemberment provisions&lt;br&gt;
          of the policy.&lt;/p&gt;
	&lt;p&gt;      6.  The benefit period is 100 months.&lt;/p&gt;
	&lt;p&gt;      7.  If the disabled individual should die during the time disability&lt;br&gt;
          benefits are being paid, the designated beneficiary will continue&lt;br&gt;
          to receive benefits until the total Principal Sum has been paid.&lt;br&gt;
          The cause of death can be other than the original injury.&lt;/p&gt;
	&lt;p&gt;VI.     BENEFICIARY DESIGNATIONS&lt;/p&gt;
	&lt;p&gt;        Under the terms of the insurance policy, benefits payable for loss&lt;br&gt;
        of life will be paid automatically to the individual or individuals&lt;br&gt;
        (in equal shares) in the order of the following categories of&lt;br&gt;
        survivors:&lt;/p&gt;
	&lt;p&gt;        A. Insured's spouse.&lt;/p&gt;
	&lt;p&gt;        B. The insured's child or children (including adopted children).&lt;/p&gt;
	&lt;p&gt;        C. The insured's parents.&lt;/p&gt;
	&lt;p&gt;        D. The insured's brothers and sisters.&lt;/p&gt;
	&lt;p&gt;        E. If none of the above is living, then the insured's estate.&lt;/p&gt;
	&lt;p&gt;        Only those eligible individuals wishing to designate a beneficiary&lt;br&gt;
        or beneficiaries different from the order given above should&lt;br&gt;
        complete a "Beneficiary Designation" form (see attachment).&lt;br&gt;
        Eligible individuals completing a form should be advised that it&lt;br&gt;
        remains in effect until it is either changed or revoked.  The&lt;br&gt;
        beneficiary designation does not automatically end with the return&lt;br&gt;
        from a business trip.&lt;/p&gt;
	&lt;p&gt;        The  completed "Beneficiary Designation" form should be forwarded to&lt;br&gt;
        the Payroll Department for placement in the eligible individual's&lt;br&gt;
        file.&lt;/p&gt;
	&lt;p&gt;VII.    SUBMISSION OF CLAIMS&lt;/p&gt;
	&lt;p&gt;        The Office of Risk Management, Office of the President, should be&lt;br&gt;
        notified as quickly as possible in the event of a claim under this&lt;br&gt;
        program.  A claim form will be issued at the time of notification of&lt;br&gt;
        loss.  However, in order to expedite processing, pertinent data,&lt;br&gt;
        including the name(s), department, nature of injury, date of death,&lt;br&gt;
        place of accident, and nature of the accident, will be taken by&lt;br&gt;
        telephone.&lt;/p&gt;
